Much as I hate to drag a thread back on course...
Went to Cafe Caldesi in Marylebone at lunchtime and had a very pleasant double espresso. I was sat in a position that enabled me to watch the preparation and passage to my side. Friendly staff even though they were fairly busy; drank up sharpish then mooched on to work very satisfied and abuzz with the caffiene. I imagine they may well take my coffee cash again very soon :D

Might be worth suggesting Sacred Cafe just of Carnaby Street. They are trying pretty hard though the espresso wasn't quite on the money, didn't see any milk drinks I am afraid.
